Director of Social Services (LCSW / LICSW)Responsibilities of the Director of Social ServicesDirects and monitors the social work services in the center.Promotes the psychosocial wellness of the residents.Actively participating in resident assessments and discharge planning to allow the resident to optimize healthcare benefits.Works closely with the Interdisciplinary Team with regards to the social wellbeing or the resident.Coordinates and arranges for resident's care planning meetings ensuring that the resident and responsible persons are involved as able.Works closely with community resources such as Home Health, Hospice, local housing authorities, shelters, and day treatment providers to ensure resources are available as needed per discharge plan.Discusses with resident and family members/responsible party regarding Advanced Directives.Thoroughly and accurately complete assessmentsSupervises the second social worker in the department.Qualifications of the Director of Social Services-Bachelors Degree in Social Work, Masters Degree preferred.Three years experience in geriatric, hospital, administrative or community social work.MUST have current Social Work licensure in MA.MUST have healthcare, preferably skilled nursing care, experience.Benefits:Competitive rates- above industry standard.Weekly pay.401(k) Retirement Planning with Company Match.A comprehensive, outstanding benefit package that includes generous vacation, holiday pay, health, dental, vision insurance.
